Tuberculosis (TB) remains one of the world's most important infectious diseases. Caused primarily by Mycobacterium tuberculosis, TB can affect the lungs and other organs and may remain clinically silent before progressing to active disease.
Early and accurate TB testing is an important component of tuberculosis prevention, diagnosis, treatment and public-health control.
Among available laboratory approaches, the TB blood test, particularly interferon-gamma release assays (IGRAs), provides an important method for detecting immune sensitization to Mycobacterium tuberculosis.
What Is a TB Blood Test?
A TB blood test is a laboratory test performed on a blood sample to determine whether the immune system has been sensitized to antigens associated with Mycobacterium tuberculosis.
The best-known category of TB blood tests is the interferon-gamma release assay (IGRA).
IGRAs measure the release of interferon-gamma (IFN-γ) from immune cells after exposure to specific M. tuberculosis-associated antigens.
How Does an IGRA TB Blood Test Work?
The biological principle behind an IGRA is based on cell-mediated immunity.
A blood sample is exposed to selected M. tuberculosis antigens. If circulating T cells have previously been sensitized to these antigens, they may respond by releasing interferon-gamma.

How Accurate Is a TB Blood Test?
The diagnostic performance of a TB blood test depends on the population being tested, the assay used, immune status, disease status and the clinical question.
Systematic reviews and meta-analyses have found that evaluated IGRAs generally demonstrate useful diagnostic performance for detecting tuberculosis infection, although performance varies between assays and populations.
Can a TB Blood Test Diagnose Active Tuberculosis?
An IGRA detects an immune response associated with Mycobacterium tuberculosis infection. It does not directly demonstrate that viable M. tuberculosis organisms are currently causing active disease.
A patient with a positive IGRA may have latent TB infection, previous TB infection or active TB disease.
Therefore, a positive TB blood test generally needs to be interpreted together with clinical evaluation and, when active disease is suspected, appropriate microbiological or molecular testing.
TB Infection vs. Active TB Disease
TB Infection
A person can be infected with M. tuberculosis without having symptoms or active disease. The immune system may control the bacteria, producing what is commonly referred to as latent TB infection.
Active Tuberculosis Disease
Active TB occurs when tuberculosis bacteria are multiplying and causing disease.
Possible symptoms of pulmonary TB include:
- Persistent cough
- Chest discomfort
- Fever
- Night sweats
- Fatigue
- Unexplained weight loss
What Happens After a Positive TB Blood Test?
A positive TB blood test does not automatically mean that a person has contagious tuberculosis.

- TB exposure history
- Symptoms
- Risk factors
- Physical examination
- Chest imaging when appropriate
- Microbiological testing when active TB is suspected
- Drug-resistance testing when TB disease is confirmed
Why Is TB Testing Important?
TB control depends on finding infections and disease early enough to interrupt transmission and provide appropriate treatment.

Molecular Tests
Rapid molecular technologies can detect M. tuberculosis and, in appropriate assays, provide information about resistance to important anti-TB drugs.
Microscopy and Culture
Microscopic examination and mycobacterial culture can contribute to the laboratory diagnosis of tuberculosis. Culture can also support antimicrobial susceptibility testing.
Imaging
Chest radiography and other imaging techniques can provide important clinical evidence when pulmonary TB is suspected.
Limitations of TB Blood Tests
- They cannot reliably distinguish latent infection from active disease.
- False-negative results can occur.
- Indeterminate results are possible.
- Performance varies between populations.
The Future of TB Testing
The global effort to eliminate tuberculosis increasingly depends on diagnostic technologies that are rapid, sensitive, specific, affordable, scalable and accessible.
Research is progressing toward improved molecular diagnostics, next-generation immunological assays, host-response biomarkers and integrated diagnostic platforms.
TB Testing For a World Free of Tuberculosis
The fight against tuberculosis is not based on one diagnostic technology. It depends on a coordinated approach connecting early testing, accurate diagnosis, effective treatment, drug-resistance detection, contact investigation, prevention and public-health surveillance.
The TB blood test represents one important part of this diagnostic landscape. When appropriately selected and correctly interpreted, blood-based immune testing can help identify people with M. tuberculosis infection and support clinical decision-making.
A world free of tuberculosis will require not only better treatments, but also faster detection, more accurate diagnostics, improved access to testing and continued scientific innovation.
